Instrumentation Amplifier with ±300V Common Mode Range and CMRR > 150dB

The differential input voltage is sampled on a flying capacitor and then its charge is transferred to a fixed capacitor with is then amplified to produce an output voltage. The use of high voltage photo driven FET switches rejects very high input common mode voltages.
